Boarding Notification
I understand that Eastlake Veterinary Services will not be responsible for any illness that my pet may contract during their stay at our facility. I also understand that if my pet has not been fully vaccinated at least one week prior to boarding/grooming, the above also applies. We also will not be responsible for any symptoms that present after leaving our facility. We do disinfect daily and strive to maintain a healthy environment for your pet.
